Extremely sustained startle-induced clonus: Non epileptic motor attacks mimicking clonic seizures in children with encephalopathy

چکیده
Clonus is a pathological motor pattern characterised by involuntary, rhythmic and brisk muscular contractions in response to peripheral stimuli producing muscle stretching. It indicates pathological involvement of the corticospinal tract and can be considered as a functional spastic movement disorder of variable clinical presentation and duration. We documented severe and prolonged episodes of startle-induced clonic attacks associated with severe apnoea, occurring in three infants with severe encephalopathy. The clinical characteristics of such episodes are very similar to those of clonic epileptic seizures. Video-EEG recordings confirmed the non epileptic origin of the episodes. Previous anti-epileptic drug treatment was unsuccessful but myorelaxing drugs produced a dramatic improvement.

Prolactin and seizure activity.
Prolactin secretion after tonic-clonic seizures (10 patients), complex partial seizures (five) and non-epileptic attacks (three) was studied in a group of children aged between 0.3 and 14 years. Seven patients with other subcategories of seizure disorders were also studied. Eight children with tonic-clonic seizures exhibited post ictal concentrations of prolactin greater than 500 mU/l. One of t...

AbstractObjectives: This research aimed at assessing the efficacy of psychological and non-medicinal treatments in controlling epilepsy of children and youths. Method: 30 boys, aged 9 to 24, with normal IQ, diagnosed with tonic-clonic epilepsy were selected as the subjects of the research. They had been under medicinal treatment for two years, but showed no adequate response. The subjects rando...
متن کاملElectrophysiology of focal clonic seizures in humans: a study using subdural and depth electrodes.
Focal clonic seizures are a frequent epileptic phenomenon. However, there are few data about their pathogenesis. Eleven patients with focal epilepsy who experienced focal clonic seizures during prolonged video-EEG monitoring were included in this study. Nine patients had subdural electrodes on the precentral gyrus and one patient had additional bilateral subthalamic nucleus (STN) depth electrod...
متن کاملSerum Prolactin Level after Febrile Seizure versus Epileptic Seizure in 6-Month-Old to 5-Year-Old Children
Background: Serum prolactin level has been used as an alternative determinant to help differentiate epileptic from non-epileptic seizures. We aimed to analyze the association between serum prolactin concentration and epileptic seizure versus febrile seizure as well as seizure duration, single versus multiple seizure attacks and time lapse between seizure onset and blood sampling. Methods: Seven...
متن کاملThe movement disorders of Coffin-Lowry syndrome.
Coffin-Lowry syndrome (CLS) is an X-linked semi-dominant condition with learning difficulties and dysmorphism caused by mutations in the gene RSK2. Originally, epilepsy was reported as a feature. We and others have since described predominantly sound-startle induced drop attacks that have been labelled 'cataplexy', abnormal startle response and hyperekplexia. We sought to clarify why there shou...
